OSHA 300 Active Report Details

DEPENDENCIES:

Before OSHA Logs can be produced from ClientSpace the following areas will need to be used and have accurate data.

  • WC Claims
  • RTW Records
  • Locations
  • Employee Vouchers

FILTERS:

  • Client: If filled in then run for that Workspace. If the report is run from within a Client workspace, this field will autofill for that client.  If the Client field is not completed the report will only run for Workspaces where the Client Master Status = Client, unless "Include Terminated Clients" (see below) is checked.
  • Location: A drop list of Active Locations in that Workspace (this field is not appropriate for use when running from the Admin Workspace).
    • The drop list on the filter only shows Active Locations.
    • When "Summarize by account" (see below) is not checked and the location is not populated, a report for every client location, including active Client Location records and inactive Client Location records where the Obsolete Date (field on the Client Location) is within the OSHA Year selected. Inactive records that don't have an Obsolete Date are excluded.. The claim will be shown on the OSHA 300 report based the Comp Claimant Location field.
  • Contract Type: Primarily used when running the report from the Admin workspace for multiple clients, this will allow you to filter for Client Workspaces of the selected contract type.
  • Year (Required): This required field filters for Claims logged in the selected calendar year.
  • Summarize by Account (checkbox): When this box is ticked, all available claims for the OSHA year selected will display under the Primary location for that client.
  • Client Options: Used when running for multiple clients from the admin workspace, select for clients With Claims, Without Claims, With and Without Claims.
  • Include Terminated Clients: Primarily used from the admin workspace, This setting allows you to include claims from workspaces where the client may have had claims and been terminated in the OSHA year selected.

Important to Note:

  • If "Suppress OSHA Logs" on the Workers' Comp Claim form is checked the Claim will not be included on this this report.
  • The OSHA Code can be configured in the Admin Workspace to not show on the OSHA 300. 
    • Navigate to the Admin Workspace, open the OSHA Code dataform and select the code in question. 
    • Uncheck the "Show on OSHA 300 Report" box to hide this code from the OSHA Log reports.
  • The OSHA Log reports (300, 300A) previously included inactive locations in the report. As some clients have a business need for inactive locations and there is no way to delete these locations through the user interface we have enhanced the Client Location form with an "Obsolete Date" field. The OSHA 300/300A logic has been modified to include active Client Location records and inactive Client Location records where the Obsolete Date is within the OSHA Year selected. Inactive records that don't have an Obsolete Date are excluded.
  • Privacy Concerns - If the "Privacy Concerns" checkbox on the Comp Claim form has been checked, the Claimant's Name on the OSHA 300 report will be replaced with the word "Private".

COLUMN MAPPINGS:

  • Column (A): The Carrier Claim Number
  • Column (B): If Privacy Consideration is checked on the Claim tab then "Private" will output, otherwise the Employee's first and last name.
  • Column (C): The Job Title will fill in automatically with the value from the Employee Employment Job Title Field.   
  • Column (D): Mapped to CompClaim.DateOfInjury field.
  • Column (E): The Accident Site from the Injury tab will show here.
  • Column (F): The Injury Accident Details from the Injury tab will show here.
  • Column (G): This will get a tick if the CompClaim.Resulted In Death field is set to yes.
  • Column (H): This will get a tick if not a Death AND a RTW record exists for Out Of Work.
  • Column (I): This will get a tick if not a Death AND there are not Out of Work RTW records AND a RTW record
  • exists for Light Duty.
  • Column (J):  This will get a tick if not a Death AND there are No Out of Work or Light Duty records AND a RTW record exists for either Full Duty or Alternate Job.
  • Column (K): Based on a RTW record of Out of Work. Not affected by Alternate Job or Full Duty
  • Column (L): Based on a RTW record of Light Duty. Not affected by Alternate Job or Full Duty
    • Note on columns (K) and (L):  If a RTW record has no End Date, then End Date is assumed to be 12/31 of the OSHA Year. If value is greater than 180 days it will just default to 180 days.
  • Column (M): Maps to the CompClaimInjury.OSHA Classification field.

Days away from work is based on Return To Work records that have a status of "Out of Work".

Job Transfer or Restrictions is based on RTW records that have a status of "Light Duty".  Intentionally, excludes "Alternate Job".

OSHA Case Number

An integer field has been added to the Comp Claim dataform for OSHA Case Number. This field is read-only and is set via auto-numbering business logic on insert of the claim form. This Auto-numbering begins at 1 at the beginning of each calendar year based on the Date of Injury (DOI) year of the claim within the Workspace. The rule is configured to run on the Default and Imports pipelines. If the Injury Date of a claim changes, a warning will be triggered to notify the user that the OSHA Case Number will be re-generated. The case will then be Auto-Numbered based on the new Injury Date. The Case Number will then be set based on the next available case number for that year.

The OSHA 300 report has been enhanced to show the Case Number and is sorted by this number. Smallest to Largest.
The Case Number will be shown if present, otherwise, it will show "Carrier's Claim Number".
The OSHA 301 Merge (Run from Comp Claim) has been enhanced to show the Case Number and is sorted by this number, Smallest to Largest. The Case Number will be shown if present, otherwise, it will show "Carrier's Claim Number".
